2025 Open House for Emerging Adults Transitional Home hosted by Urban Triage February 27th

February 24, 2025

A Vision Rooted in Empowerment

Urban Triage's Emerging Adults Transitional Home stands as a testament to our unwavering commitment to Madison's youth. Born from our mission to support those facing systemic barriers, this home offers more than smaller—it provides a foundation for growth, independence, and success. Our work is inspired by the lived experiences of our community, including Urban Triage's founder, Brandi Grayson, who understands firsthand the challenges faced by young adults navigating life after foster care.

Comprehensive Support for a Brighter Future

The Emerging Adults Transitional Home delivers a full range of services designed to equip youth with the tools they need to thrive:

  • Life Skills Development: Hands-on workshops covering personal finance, job readiness, and educational advancement.
  • Trauma Recovery and Mental Health Support: Access to counseling and wellness services to promote healing and resilience.
  • Administrative Assistance: Support with obtaining vital documents, such as birth certificates and social security cards.
  • Educational Opportunities: Partnerships with Madison College offer free career training, including CNA and phlebotomy certification programs.

Empowering Independence

Urban Triage goes beyond traditional housing support. As part of our commitment to self-sufficiency, the home will be fully furnished with items that residents can take with them when they transition to independent living—furniture, electronics, and household essentials—ensuring they leave with the tools to create their own homes.

Transforming Lives, One Step at a Time

This initiative is more than a housing solution—it's a pathway to empowerment. By offering stability, skill-building, and community support, the Emerging Adults Transitional Home will have a lasting impact on the lives of Madison's youth, breaking cycles of instability and creating new opportunities for success.
