Community Support Services
Can I still receive assistance if I am months behind in my rent or bills?
When applicants are more than one month behind in their rent or bills, we set up a one-on-one intake to evaluate and assess the applicant's situation and determine which community supports are best for the applicant.
How does Urban Triage, Inc. support applicants in accessing rental assistance available through other organizations?
Through our direct referral relationships with Tenant Resource Center and Dane County Collaborations of Black Service Providers, we are able to support our applicants with accessing other community assistance and supports.
Can I still receive assistance if my name isn't on the lease or the bill?
When applying for rental assistance, we are only able to provide assistance to individuals who are currently on the lease. If you are in a situation where you need rental assistance and your name isn't on the lease, we encourage you to have the lease holders apply on your behalf.
What do I do if I haven't received or do not have the documentation necessary to complete the application?
To complete your application process, you have three (3) business days to submit the requested documentation. If we do not receive the requested documentation in that time frame, your application will be considered void and you will have to reapply. If you need support in obtaining the paperwork please communicate that to the Program Specialist. If you are not in communication with the Program Specialist, your application will close. The timeline to submit documents can be extended, which will be based on individual circumstances.
How long does it take for Urban Triage, Inc. to process applications for assistance?
Processing applications for assistance can take approximately 3 to 7 business days.
How will I be notified about my application for assistance?
You will receive an email immediately confirming your application has been received.
If all required documentation has been received with the application, a Program Specialist will reach out to the landlord or merchant to verify the information provided. An email will then be sent to the applicant confirming or denying acceptance.
If all documentation has not been received, a Program Specialist will make contact with the applicant within 2 business via email and/or phone. The applicant will have 3 business days from that date to submit all requested documentation.
What are some reasons my application could be denied?
When applying for assistance with us, there are a number of factors that could cause your application to be denied. Some of these reasons include, but are not limited to:
- If the contribution from Urban Triage, Inc. will not prevent eviction
- Urban Triage, Inc. is out of funds
- Lack of responsiveness from applicant
- If payment arrangements can not be made with landlord
- If an applicant has received similar support in the last 90 days from Urban Triage, Inc.
- If applicant has provided false information or documentation in the last 12 months
- If someone at the same address has received support in the last 90 days
- If the applicant is not on the lease
- Incomplete applications
How much rental assistance is available per household?
Urban Triage, Inc. can provide rental assistance up to $400 per household, per 90 days.
What required documentation do I need to provide with my application for assistance?
When applying for assistance with Urban Triage, Inc., we require the following documentation to be submitted with your application:
Rental Assistance Documentation
- Copy of lease
- Documentation of any payment arrangements
- W9 from landlord
- Verification of amount owed
Past Due Bills Documentation
- A copy of the most recent bill (last 30 days) with the applicant's name on it
How often can I receive assistance/support?
Applicants can receive assistance through Urban Triage, Inc.'s programs every 90 days.
What areas in Dane County does Urban Triage, Inc. support?
We primarily provide assistance to residents of the Cities of Madison and Fitchburg. However, there may be grants or funding that we receive which allow us to extend our reach throughout Dane County. If you live outside of Madison or Fitchburg, we recommend still reaching out to us to see if we have funds available for your residency location.
What happens if the applicant's information is found to be false?
If an application is found to contain fraudulent information, the applicant and anyone associated with the same address will be restricted from applying with Urban Triage, Inc. for any support or services for 12 months.
Who is eligible to apply for support and/or services from Urban Triage, Inc.?
In order to be eligible for support and/or services from Urban Triage, Inc., the applicant must:
- be 18 years or older;
- have their name on the lease or bill;
- show the ability to pay rent moving forward, or be able to show proof of ability to cover payment arrangements; and
- submit all requested documentation.
What happens if an applicant is unemployed or underemployed?
If you are unemployed or underemployed and applying for assistance with Urban Triage, Inc., you must show that they are taking steps towards becoming employed or vocational development.
What are Urban Triage, Inc.'s operating hours?
Urban Triage, Inc. operates three (3) of the four (4) weeks of the month (the 1st of the month through the 21st of the month). We begin operations again on the 1st of the following month.
During the first three (3) weeks of the month that we are in operation, we are available Monday through Wednesday, from 9:00 am to 3:00 pm. On Thursdays and Fridays, we process applications. We are closed on the weekends.
