Healthy Youth
Cultivating Healthy Youth
Urban Triage empowers youth and families through housing stability, education, advocacy, and systems navigation, providing transformative, wraparound support that moves people from crisis to long-term stability and self-determination.

Real Support for Youth
Urban Triage’s youth programming is youth-led, relationship-driven, and rooted in the real needs of our community. We don’t design programs for young people—we build them with them. That means listening first, showing up consistently, and creating spaces where youth are not just served, but truly seen, heard, and valued.

Meeting Young People Where They Are
Our work supports youth navigating housing instability, involvement in systems, and major life transitions through wraparound support that meets them where they are. This includes mentorship, leadership development, life-skills education, systems navigation, emotional wellness support, and pathways to stability and self-determination. Whether youth are transitioning out of foster care, experiencing homelessness, or seeking a sense of belonging and direction, Urban Triage provides steady support, trusted relationships, and opportunities to lead.
We believe youth are experts in their own lives. Our role is to walk alongside them—removing barriers, strengthening protective factors, and helping young people build the skills, confidence, and community they need to thrive.

Youth Drop-In Hours
Urban Triage's Youth Drop-In Hours are designed for youth ages 14 to 22 experiencing homelessness, fleeing domestic violence, needing job readiness support and placement, or simply needing a safe, supportive environment.

School Partnerships
Our Curriculum is More Than Just an After-School Program
It helps youth ages 14 to 18 build self-awareness, emotional resilience, and meaningful connections through a mix of personal development, social connection, and one-on-one case management.

More than housing. A real home.
Emerging Adults Home
The Emerging Adults Home provides a supportive, stable living environment for young adults ages 18–22 who are transitioning out of court-ordered placement. This home is designed to support growth, accountability, and independence during a critical life transition.

Residents receive up to 20 hours of on-site support daily, including relationship-based case management, life-skills development, and emotional wellness support. Our approach centers dignity, choice, and real-world preparation—helping young adults build self-awareness, emotional resilience, and meaningful connections while working toward long-term stability.
Urban Triage walks alongside residents as they strengthen daily living skills, navigate systems, and prepare for independent adulthood—at a pace that supports lasting change, not just compliance.
