We provide transformative education, housing support, and direct services designed to empower individuals and families to heal, thrive, and lead within their communities. Our work centers on self-sufficiency, self-knowledge, and systemic change.

We operate from a lens of radical Black love and community accountability. We meet people where they are — with compassion, honesty, and resources that create real change. Our programs don’t just offer aid; they build skills, confidence, and independence.

We serve individuals and families — primarily in Dane County — who are experiencing or at risk of homelessness, housing instability, or other systemic barriers. We focus on Black and marginalized communities while welcoming anyone seeking support or partnership.

Coordinated Entry is Dane County’s system for connecting people experiencing homelessness to housing and support services. Urban Triage is one of the access points where individuals and families can complete their housing assessment and receive referrals.

You can contact our Housing Support Team or visit our Housing Resources page. We’ll help assess your situation, connect you to available resources, and support you through the Coordinated Entry process.

Yes. We offer eviction prevention, rental assistance, and case management services. If you’ve received an eviction notice, reach out immediately — the sooner we connect, the more options we have to help stabilize your housing.

Not always. Coordinated Entry prioritizes households with the highest needs. We’ll support you through the process and connect you to other resources (shelters, rental assistance, or emergency support) while you wait.
