Outreach

Our Outreach Specialists are dedicated to visiting neighborhoods weekly, engaging directly with individuals who face homelessness.

Connecting People to Essential Support

They guide individuals through the housing process, provide assessments, essential needs, and connect them to various services.

Collaborating with Local Organizations

This proactive engagement thrives on partnerships with local organizations, including the Homeless Consortium.

Bringing help Where Life Happens

It encompasses outreach activities like pop-up support events at libraries, parks, community centers, churches, food pantries, and barbershops.

Spreading the Word, Sharing Resources

We also distribute flyers to create awareness and provide direct access to support.

Partnering with Landlords to Provide Permanent Housing

Urban Triage collaborates with landlords and property management companies to provide permanent housing options to individuals and families.

Housing Navigation Services

Urban Triage's Housing Navigation Specialists are responsible for leading our Housing Navigation Services at the Beacon Day Shelter and other community spaces, including the Salvation Army.

In collaboration with Dane County, we strive to provide compassionate, caring, and culturally sensitive services to individuals and families experiencing homelessness, as defined by HUD, which includes residing on the streets, in a car, or a shelter.

Wrap-Around Support

We provide individualized support and guidance through intensive case management to help community members navigate various systems and access necessary resources. Additionally, we offer:

Advocacy

Representing the needs and interests of youth and families within different systems (e.g., schools, justice system).

Trauma Recovery

Addressing the impacts of trauma through various therapeutic approaches, including mindfulness practices, yoga, and somatic work.

Education and Skill Building

Providing career development, financial literacy, social-emotional learning, and leadership training to foster self-sufficiency and empowerment.

Housing Stability Support

We provide holistic support to prevent individuals and families from becoming homeless.

When funds are available, we provide direct financial intervention to help families avoid eviction and maintain their housing stability. This serves as a crucial safety net, preventing vulnerable community members from falling into homelessness.

We provide one-on-one advocacy on behalf of community members to help them maintain their housing stability by representing their needs within social systems and in negotiations with landlords to resolve disputes that can lead to eviction.

Often, community members face a complex and compounding set of barriers that threaten their housing stability. Our transformational case management services work to connect individuals with resources, support, and coaching to help them overcome these barriers.

Supporting Healthy Black Families

We offer educational programs based on our personal transformation curriculum, with each cohort tailored to specific audiences.

Healing, Resilience, and Power

SHBF Workgroups

Our Supporting Healthy Black Families (SHBF) work groups address health disparities through education, resources, and advocacy. Through Black-centered principles, teachings, and a multi-disciplinary curriculum, participants learn to navigate grief and adversities, including systemic racism.

Because Breaking Cycles Starts in the Mind

Cohorts meet once or twice a week for up to 6-16 weeks, learning about the fundamentals of perception psychology, how our minds function, how assumptions are developed, how habits are formed, and how to change them.

Generational Curses End Here

We equip individuals with the tools to break generational cycles and curses, and we offer new language and insights that empower and inspire transformation from within.

Healing Through Shadow Work and Inner Growth

Past participants have shared that the curriculum provided guidance to help them overcome depression, isolation, and procrastination, as well as escape abusive situations by doing the inner work, what we call shadow work.

Grow Together, Heal Together

Supporting Healthy Black Elders

Our Supporting Healthy Black Elders workgroup is designed for Black individuals aged 55 and older. Participants engage with our transformational curriculum through weekly sessions and enjoy enriching community activities, such as painting classes, farmers' markets, outdoor plays, ceramic workshops, and outdoor concerts, once a month.
